Below please find the informational letters and registration forms needed to register your child(ren) for our Faith Formation Program:

**First Grade Faith Formation Registration will take place on Sunday, September 15th. Children and their families are invited to the 11:00 a.m. Mass. Following Mass, please join us in the school gym for Ice Cream Sundae Sunday! While your child (and you) enjoy some delicious ice cream, you can register your child for our two-year preparation program for First Holy Communion. Please provide a copy of your child's Baptismal Certificate. You may contact the Office of Faith Formation at 631-589-3160 for more information.
